Ask for the medicine’s name, the person making the care decision and a bill with every charge.
Keep the product name, the care plan and the bill on the same page.
Are peptides real medicines?
Some peptides are medicines. A peptide is a tiny chain made of parts linked together. The parts used in that chain also make up proteins.
The US Food and Drug Administration (FDA) approves particular products for particular uses.
Approved examples, as of September 26, 2026, are semaglutide under the names Ozempic and Wegovy, and tirzepatide under Mounjaro and Zepbound.
FDA approval applies to the exact product. A custom-made version does not share that approval, even when it uses the same main ingredient.
Ask which medicine is proposed, what form it comes in and why it would be used.
What does compounding mean?
In compounding, a pharmacist or doctor makes medicine to suit a particular need instead of providing a ready-made product.
Compounded medicines had no FDA approval as of September 26, 2026.
Before these medicines are supplied, the FDA does not assess their safety, their quality or whether they work well.
According to the FDA, a custom medicine may serve a need left unmet by an approved product.
Making a custom medicine can be legal when it meets both state and federal rules.
Federal law limits pharmacies and doctors making regular copies, or large numbers of copies, of ready-made medicines. This includes copies that are the same or nearly the same.
The person who writes your medicine order is called the prescriber. This copy rule has an exception for a change that person decides makes an important difference for your own medical need.
Ask: what need of mine does a ready-made medicine fail to meet? What is changed in the custom version, and what makes that change matter for my care?
Can a visit start online?
Telehealth is care through video, phone or an online form.
The clinician is the health professional providing the care.
Arizona rules, checked on October 3, 2026, do not require an in-person visit before each remote prescription.
The clinician must still examine you using a method suited to your needs and follow the rules for that medicine.
Arizona requires the clinician to decide whether care from a distance fits your needs, as of October 3, 2026.
The Federation of State Medical Boards brings together boards that oversee doctors. Its advice says a fixed set of questions alone is not enough to order medicine.
That advice calls for a way to ask more questions about your answers. It gives doctors a standard to follow; it is not a law.
Ask who reviews the answers and how to speak with that person. Keep the names of current medicines and your main questions handy.

What might I pay?
Cost examples as of September 26, 2026.
Cash for the medicine
For weight and diabetes medicines, the cash prices makers advertised were mostly about $149 to $499 per month.
The product, strength and refill timing can change the price; some late refills cost more.
A price “per month” can cover different amounts: a box of pens, a bottle of tablets or 28 days of medicine.
Ask: what does my exact product cost after any opening offer?
The care service
In a small group of online weight services checked, most advertised membership fees that repeat. Medicine cost extra.
Ask: are visits, tests, medicine and shipping included? What keeps being charged, and how do I cancel?
A medicine price and a total care bill are different figures. Ask for both in writing.
If Medicare is paying
A temporary Medicare programme, as of September 26, 2026, offered certain weight-loss medicines to some people with a $50 monthly copay.
The copay is the part the patient pays.
The programme requires Medicare prescription-drug coverage, called Part D. This may be a separate drug plan or part of a Medicare Advantage plan with drug coverage.
Medicare by itself does not make someone eligible. To qualify, you must meet the programme’s health and drug-plan rules and be at least 18.
The stated programme window is July 1, 2026, through December 31, 2027.
A prescription and approval of the request are required. Weight, other health problems and past payments by your Medicare drug plan affect whether you qualify.
In 2026, people were excluded from the programme if their Medicare drug plan had paid for a GLP-1 medicine earlier that year. GLP-1 is the name of a group of medicines used for weight or diabetes care.
Type 2 diabetes is excluded from this programme, under the September 26, 2026 rules.
For type 2 diabetes, requests go through the drug plan’s usual medicine coverage.
Covered medicines under your Medicare drug plan have a yearly limit on the amount you pay. The $50 payments for this programme are left out when adding up payments toward that limit. These payments do not move you nearer to the point when the plan no longer charges for covered medicines. Coupons cannot reduce the $50 payment.
Ask your Medicare drug plan and the person writing your prescription: does my exact medicine qualify for this $50 price? Do I qualify, or does my request belong under the plan’s usual coverage? For other insurance, ask whether it covers the exact product for your medical need.
In FDA letters checked on September 26, 2026, products marketed for human treatment were unapproved drugs despite their research labels.
As of September 26, 2026, the FDA states that using retatrutide or cagrilintide in compounding is barred by federal law.
Ask for the exact medicine name and its approval record. A label or a sales claim cannot answer for the clinician.
How is care handled after the first visit?
Ask who handles questions, how soon to expect a reply and how the next review is arranged.
The medical boards’ advice calls for the doctor to arrange needed tests. If an exam is needed in person, the doctor must arrange care with someone who can do it.
Ask how that would work for you before agreeing to a plan.
